Semaglutide vs Tirzepatide vs Retatrutide: Which GLP-1 for Which Goal

Three compounds, three generations of the same idea, and a fairly steep gradient of results between them. In their pivotal trials semaglutide produced around 15% body-weight loss, tirzepatide around 21%, and retatrutide around 24%. Those numbers are not marketing spread - they track a real progression from hitting one hormonal receptor, to two, to three.

That progression is the whole story here. Each drug adds a receptor target, and each added target changes not just how much weight comes off but the side-effect profile, the cost, and how mature the safety data is. The newest and most powerful option is also the least proven. The most proven is the weakest. Deciding between them is really a decision about where on that curve your own goals and risk tolerance sit.

The incretin system, briefly

After you eat, your gut releases hormones called incretins that tell the body a meal has arrived. Two matter here: GLP-1 and GIP. Its weight-loss evidence comes from the STEP trial programme, where participants on the 2.4mg weekly dose lost roughly 15% of body weight over 68 weeks - a result no previous non-surgical intervention had come close to.

The mechanism is appetite-led. By activating GLP-1 receptors in the brain and slowing gastric emptying, semaglutide reduces hunger and makes smaller meals satisfying. Most people simply eat less without the constant fight against appetite that sinks conventional dieting.

It is dosed once weekly by subcutaneous injection, titrated up slowly over months to manage side effects. Those side effects are overwhelmingly gastrointestinal - nausea, constipation, occasional vomiting - worst during dose increases and usually settling with time and slower titration.

Where semaglutide wins is maturity and predictability. It has the longest track record, the largest body of safety data, and the widest availability. If the goal is meaningful weight loss with the most established option and cost matters, it remains a completely reasonable first choice. It is simply no longer the most powerful.

Tirzepatide: the dual agonist that changed the benchmark

Tirzepatide adds GIP agonism on top of GLP-1, and the combination outperforms GLP-1 alone by a clear margin. In the SURMOUNT-1 trial, participants on the top 15mg weekly dose lost about 21% of body weight over 72 weeks, with a substantial fraction losing more than a quarter of their starting weight.

The reason the dual approach works better isn't fully settled, but GIP appears to do two useful things: it complements GLP-1's metabolic effects, and it seems to reduce the nausea burden relative to the amount of appetite suppression achieved. Many users report that tirzepatide feels somewhat smoother than semaglutide at comparable levels of appetite reduction, though GI side effects are still the main complaint and still peak during titration.

Dosing mirrors semaglutide - once weekly, titrated over months - and its glycaemic control is strong enough that it has become a leading option for type 2 diabetes as well as weight loss.

Tirzepatide is, for most people in 2026, the sweet spot: markedly stronger results than semaglutide, a large and growing evidence base, approved status, and a tolerability profile that holds up. Where it wins is the balance of power and proof.

Retatrutide: the triple agonist on the frontier

Retatrutide adds glucagon receptor agonism to the GLP-1/GIP combination, making it a triple agonist. In its Phase 2 trial, participants on the 12mg dose lost roughly 24% of body weight at 48 weeks - and notably the curve had not clearly plateaued, suggesting the ceiling may be higher still.

The glucagon component is what distinguishes it. Where semaglutide and tirzepatide work primarily by reducing intake, retatrutide adds an energy-expenditure and hepatic-fat mobilisation effect on top. That is a mechanistically different way to lose weight - burning more rather than only eating less - and it is the reason the results run ahead of the dual agonist.

The tradeoffs are real and follow the pattern. Retatrutide is still investigational, without the completed long-term outcome and safety data that semaglutide and tirzepatide have accumulated. Its GI side-effect profile is at least as demanding, and the glucagon activity means blood-sugar and cardiovascular effects warrant closer attention. It is the most powerful option and the least established, in the same breath.

Where it wins is maximum efficacy for someone who is comfortable being early to a compound whose long-term profile is still being written.

Semaglutide makes sense if you want solid, proven weight loss with the most mature safety data, or if cost and availability are the deciding factors. A 15% reduction is life-changing for most people, and there is something to be said for the option that has been studied the longest.

Tirzepatide makes sense if you want materially stronger results than semaglutide without stepping onto the frontier - and especially if you also have metabolic or glycaemic issues, where its dual mechanism shines. For the majority of people asking this question, tirzepatide is the right answer.

Retatrutide makes sense if maximum weight loss is the priority and you accept that you are ahead of the full long-term data. It is the choice for someone who has plateaued on the others or who wants the highest ceiling and will manage the tradeoffs deliberately.

The honest conclusion

For most people, tirzepatide is the current best balance of efficacy and evidence: it beats semaglutide meaningfully while resting on a large, approved dataset. Semaglutide remains the sensible, well-proven, more affordable entry point. Retatrutide is the frontier - the strongest results by the numbers, and the least settled safety picture.

Two things are true regardless of which you pick. First, these drugs manage appetite while you take them; the weight tends to return if you stop without having rebuilt eating and activity habits, so the exit plan matters as much as the drug. Second, potent weight loss strips lean mass alongside fat unless you defend it with adequate protein and resistance training. The compound is the easy part. Keeping the result - and keeping it as muscle rather than water and lean tissue - is the work.
